Position Overview

Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a Home Health assignment with CarePartners Home Health.

This role involves providing therapy services directly in patients' homes, focusing on improving mobility, independence, and quality of life through individualized care plans.

Facility Information

• Facility: CarePartners Home Health
• Location: Asheville, NC (Franklin / Macon County area)
• Setting: Home Health
• Charting System: HomeCare HomeBase (HCHB)
• Caseload: 5–6 patients per day
• Contract Length: 13 Weeks

Key Responsibilities

• Conduct in-home patient evaluations
• Develop and implement individualized treatment plans (POC)
• Provide therapy for:
  • Mobility and strength deficits
  • Post-surgical recovery
  • Functional rehabilitation
• Assess home environment and safety risks
• Deliver:
  • Gait training
  • Balance and strengthening exercises
  • Functional mobility training
• Educate patients and caregivers on home exercise programs
• Maintain accurate documentation in HCHB EMR
• Communicate patient progress with physicians and care teams

Required Qualifications

Active North Carolina PT License
BLS Certification
Home Health experience (MANDATORY)
Experience with HomeCare HomeBase (HCHB)
Valid driver's license & reliable vehicle

Preferred Qualifications

Hospice experience
Strong OASIS documentation skills
Prior travel experience

Compliance Notes

Reliable transportation required
