Schedule: Day Shift | 8:00 AM – 5:00 PM | 40 Hours Per Week

Position Overview

Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health assignment in Stockton, CA.

This is a field-based role where the therapist will provide in-home rehabilitation services across the Central Valley/Stockton territory. The therapist will focus on improving patient mobility, restoring function, and enhancing overall quality of life through individualized care plans.

Key Responsibilities

  • Conduct comprehensive patient evaluations in the home setting

  • Develop and implement individualized treatment plans

  • Provide therapeutic exercises and functional mobility training

  • Utilize manual therapy techniques and rehabilitation modalities

  • Educate patients and caregivers on home exercise programs and safety practices

  • Monitor patient progress and adjust treatment plans accordingly

  • Maintain accurate and timely clinical documentation

Care Setting

  • Home Health (Field-Based)

  • Coverage area includes Central Valley / Stockton territory

  • Productivity-based model: 25 visits = 40 hours guaranteed

Required Qualifications

  • Minimum 1 year of Home Health experience

  • OASIS experience preferred (may consider strong HH background without it)

  • Active California Physical Therapist License (or pending with proof of application)

  • BLS Certification (AHA)

  • Valid driver's license and reliable transportation

  • Strong communication and documentation skills

Preferred Qualifications

  • Previous travel experience

  • Ability to manage an independent caseload

Assignment Details

  • Schedule Options: Tuesday–Saturday or Wednesday–Sunday

  • 40 hours guaranteed based on completed visits

  • Mileage reimbursement (IRS standard)

  • On-call rotation may be required (discuss during interview)

  • Modules completed during orientation (billable)

  • Local candidates accepted at travel rate

  • Candidates must submit RTO at time of application

  • Home Health checklist required

  • CA license preferred at submission (pending accepted with proof of application)
