Position: Physical Therapist (SNF Setting)
Salary Range: $45.99-$50.54 per hour
Schedule: Full-time, 40 hours per week, Monday-Friday with no weekend rotation
Facility: 111 Bed Skilled Nursing Facility
We are currently seeking a highly skilled and dedicated Physical Therapist to join our team at our 111 bed Skilled Nursing Facility. As a Physical Therapist, you will work with a diverse patient population to provide individualized treatment plans and help patients achieve their maximum potential for physical function and independence.
Key Responsibilities:
- Conduct comprehensive evaluations to assess patients' physical abilities and limitations
- Develop and implement individualized treatment plans based on patients' needs and goals
- Utilize a variety of therapeutic techniques and modalities to improve patients' strength, mobility, balance, and overall physical function
- Collaborate with other healthcare professionals to ensure coordinated and effective care for patients
- Educate patients and their families on proper techniques for performing exercises and activities at home
- Document patient progress and maintain accurate records in accordance with facility and regulatory guidelines
- Participate in interdisciplinary team meetings and contribute to patient care planning and discharge planning
- Stay up-to-date on current trends and best practices in physical therapy through continuing education and professional development opportunities
Qualifications:
- Valid state license to practice as a Physical Therapist
- Minimum of 1 year of experience in a SNF setting preferred
- Strong clinical skills and knowledge of therapeutic techniques and modalities
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and patient-centered approach to care
- Basic computer skills and ability to learn and use electronic medical record systems
