Position Overview

Trinity Healthcare Talent Services is seeking a Travel Occupational Therapist (OT) for a home health role in Rockford, IL.

This position focuses on delivering therapy services directly to patients in their homes, supporting independence and functional outcomes.

Requirements

• Active Illinois OT License

• BLS Certification (AHA)

• Minimum 1 year of OT experience required

• Home health experience required

• Must be comfortable with independent field work

Assignment Details

• Schedule: Monday–Friday | 8:00 AM – 5:00 PM

• Rotating weekends required

• Duration: 13 weeks

• Setting: Home Health / Home-Based Primary Care

• Mileage: Reimbursed between patient visits

Facility Overview

• Independent caseload management
