Position Overview

We are seeking a Travel Physical Therapist Assistant (PTA) for a Home Health assignment in the San Jose, California area. The PTA will provide in-home therapy services under the supervision of a licensed Physical Therapist, delivering care in accordance with approved treatment plans and patient-specific goals.

This role requires strong independence, organization, and the ability to manage a home health caseload throughout the South Bay region.

Key Responsibilities

• Implement physical therapy treatment plans in patients homes

• Provide therapeutic exercises and modalities as directed by the supervising PT

• Document all patient interactions and treatment progress accurately

• Collaborate with interdisciplinary team members

• Educate patients and caregivers on home exercise programs and safety techniques

• Maintain compliance with agency, state, and federal regulations

Required Qualifications

• Active California PTA License (in hand at time of submission)

• Minimum 2 years of PTA experience – REQUIRED

• Home Health experience – REQUIRED

• BLS Certification (AHA or American Red Cross required)

• Ability to meet 50-mile travel requirement
