Position Overview

Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health assignment covering the Berkeley / Oakland area.

This is a field-based role working directly with patients in their homes, focusing on improving functional mobility and independence.

Key Responsibilities

• Perform in-home patient evaluations and develop treatment plans

• Deliver skilled physical therapy services in a home health setting

• Complete accurate and timely OASIS documentation

• Monitor patient progress and adjust care plans accordingly

• Educate patients and caregivers on exercises and safety

• Maintain strong communication with interdisciplinary team

Requirements

• Active California PT License (or pending with application)

• Minimum 1 year of Home Health experience REQUIRED

• OASIS experience REQUIRED

• BLS Certification (AHA)

• Valid driver's license and reliable transportation

Assignment Details

• Schedule options: Tues–Sat or Wed–Sun

• 25 visits = 40 hours guaranteed

• Mileage reimbursed (IRS standard)

• Territory: Berkeley / Oakland area

• Locals accepted
