Travel Physical Therapist (PT) – Home Health

Estimated Weekly Pay: $1,907.84
Schedule: 7:30 AM – 6:00 PM | ~40 hrs/week

Position Overview

We are hiring a Home Health Physical Therapist (PT) to provide patient-centered care in home settings across Macon County, NC.

This role involves delivering therapy directly in patients' homes, focusing on restoring mobility, improving function, and supporting independence—primarily in geriatric and post-acute populations.

Work Setting

  • Setting: Home Health (patient homes, ALF)
  • Coverage Area: Macon County, NC
  • EMR: Cerner
  • Team: PT, PTA, OT, RN, CNA, Social Workers

Key Responsibilities

Evaluation & Assessment

  • Perform comprehensive home health assessments
  • Complete OASIS documentation (required)
  • Evaluate mobility, strength, and functional limitations

Treatment & Care Planning

  • Develop and implement individualized care plans
  • Provide:
    • Therapeutic exercises
    • Gait and balance training
    • Functional mobility training
  • Treat conditions such as:
    • Post-surgical (THA/TKA)
    • Chronic illness
    • Geriatric mobility limitations

Patient Management

  • Monitor progress and adjust treatment plans
  • Provide patient and caregiver education
  • Ensure continuity of care

Team Collaboration

  • Supervise PTA services
  • Coordinate with interdisciplinary team
  • Participate in care planning discussions

Documentation

  • Maintain accurate, timely clinical documentation
  • Ensure compliance with regulatory standards

Schedule & Workload

  • Monday – Friday (primary schedule)
  • Office hours: 8:00 AM – 4:30 PM
  • Approx. 6 patient visits/day
  • Weekend coverage: Occasional only (not routine)

Requirements

Must Have

Active North Carolina PT License
Home Health experience
OASIS documentation experience (MANDATORY)
BLS/CPR Certification

Preferred

Knowledge of PDGM model
Geriatric care experience
Strong patient communication skills
