Position Overview

Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health assignment covering Hayward and Fremont, CA.

This role focuses on delivering one-on-one care in a home health setting, helping patients improve mobility, function, and independence in their home environment.

Key Responsibilities

• Evaluate and treat patients in a home health setting

• Develop and implement individualized treatment plans

• Perform functional mobility training and therapeutic interventions

• Complete OASIS documentation and maintain accurate records

• Educate patients and families on care plans and home exercises

• Coordinate care with interdisciplinary teams

Requirements

• Active California PT License (or pending at submission)

• BLS Certification (AHA)

• Minimum 1 year of Home Health experience

• OASIS experience preferred (required or strong HH experience considered)

• Valid driver's license and reliable transportation

Assignment Details

• Schedule options: Tues–Sat or Wed–Sun

• 25 visits = 40 hours guaranteed

• Mileage reimbursed (IRS standard)

• Locals accepted at travel rate

• Coverage area: Hayward / Fremont

• On-call rotation may be required
