Estimated Weekly Pay: $2,311.18
Schedule: Day Shift | 5x8 Hours (8:00 AM – 5:00 PM)
Possible Work Schedules:
Tuesday – Saturday
Wednesday – Sunday
Coverage Territory: Palmdale / Lancaster, California
Position Overview
Neogen Care is seeking a Travel Physical Therapist (PT) to provide skilled home health therapy services throughout the Palmdale and Lancaster territories.
The Physical Therapist will deliver patient-centered rehabilitation care in the home setting, focusing on improving mobility, functional independence, and overall quality of life for patients recovering from illness, injury, or surgery.
This role requires strong clinical judgment, time management skills, and the ability to work independently while coordinating care with interdisciplinary healthcare teams.
Key Responsibilities
Conduct comprehensive patient evaluations in the home setting
Develop individualized treatment plans based on patient needs and goals
Provide therapeutic exercises and mobility training
Monitor patient progress and perform reassessments
Educate patients and caregivers on home exercise programs and safety practices
Maintain accurate and timely documentation in the EMR system
Coordinate with physicians, nurses, and other healthcare professionals to ensure continuity of care
Workload Expectations
25 completed visits per week = 40 hours
Hours are guaranteed once 25 visits are completed
Mileage between patient homes is reimbursed according to IRS standard mileage rates
Required Qualifications
Minimum 1 year of Home Health Physical Therapy experience
Active California Physical Therapist License (or pending license with proof of application)
BLS Certification (AHA)
Valid Drivers License and Personal Transportation
Preferred Qualifications
OASIS documentation experience preferred
Strong home health clinical assessment skills
Excellent documentation and communication skills

Locals accepted at travel rate
On-call rotation may be required (discussed during interview)
Requested time off must be submitted during application
Home Health checklist completion required prior to start
Travel pairs accepted but must work in separate territories with individual transportation
