Schedule: ~34 Hours/Week
Position Overview
We are seeking a Physical Therapist (PT) to provide care across inpatient, outpatient, and possible home health settings.
The PT will evaluate, plan, and deliver treatment programs aimed at restoring mobility, reducing pain, and improving overall patient function and independence.
Schedule Details
- Monday & Tuesday: 7:30 AM – 5:00 PM
- Thursday & Friday: 8-hour shifts
- Weekly Hours: ~34 hours
- Work-Life Balance: 3-day long weekend every week
Key Responsibilities
Evaluation & Assessment
- Perform patient evaluations based on physician orders

Assess:
- Range of motion (ROM)
- Strength and functional mobility
- Neuromuscular status
Treatment & Therapy
- Develop individualized treatment plans

Utilize modalities such as:
- Ultrasound
- NMES
- Iontophoresis

Provide:
- Manual therapy
- Therapeutic exercises
- Mobility training
Care Planning & Progress Monitoring
- Set short- and long-term goals
- Monitor patient progress and modify treatment plans
- Communicate updates to physicians
Team Collaboration
- Supervise and guide PTAs
- Coordinate with interdisciplinary teams
- Educate patients and families
Documentation & Compliance
- Maintain accurate and timely documentation
- Ensure compliance with regulatory and facility standards

Requirements
Must Have
Active Illinois PT License
BLS Certification
Valid Driver's License + Auto Insurance
Strong documentation & patient care skills
Preferred
1+ year PT experience
APTA Membership
Multi-setting experience (IP/OP/Home Health)
