Position Overview
Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health setting in Dickinson, ND.
This is a field-based role where the Physical Therapist will provide in-home rehabilitation services to patients recovering from surgeries, injuries, and chronic conditions. The focus is on improving mobility, restoring independence, and delivering patient-centered care through individualized treatment plans.
The therapist will work independently while maintaining communication with clinical leadership and interdisciplinary teams.

Clinical Coverage Areas
Therapists will provide care for:
Post-surgical recovery
Fall-related injuries
Mobility and functional impairments
Home health patients with occasional hospice support visits
Strength, balance, and endurance training
Key Responsibilities
Perform in-home patient evaluations and functional assessments
Develop individualized treatment plans based on patient needs
Conduct start of care, recertifications, and resumption of care visits
Deliver therapeutic exercises, mobility training, and rehabilitation interventions
Manage a caseload of approximately 5–6 patients per day
Monitor patient progress and adjust treatment plans accordingly
Educate patients and caregivers on home exercise programs and safety
Maintain accurate and timely clinical documentation
Coordinate care with interdisciplinary healthcare teams
Manage travel routes and scheduling efficiently
Required Qualifications
Minimum 1 year of Home Health Physical Therapist experience
Active North Dakota Physical Therapist License
BLS Certification
Valid driver's license and auto insurance
Strong communication and documentation skills
Preferred Qualifications
OASIS experience preferred
Previous travel experience
Ability to work independently in a field-based environment
Scheduling & Assignment Details
40 hours guaranteed per week
Monday – Friday schedule
Shift: 8:00 AM – 5:00 PM
No weekends
No holiday requirements
Important Notes
Productivity expectation: ~6.5 points per day
Typical caseload: 5–6 visits/day (fewer visits on admission days)
May occasionally assist with hospice-related evaluations or teaching visits
No PTA support currently available
Orientation period: 1–2 weeks with preceptor
Radius Rule: Candidates must reside outside 50 miles and must not have worked with CommonSpirit/CHI/Dignity facilities in the past 12 months
Requested Time Off (RTO) must be submitted upfront (no changes allowed after submission)
Facility may cancel up to 4 shifts during the 13-week assignment
Interview Process: Manager interview only (no clinical interview)

Active state licensure verification
BLS certification
