Position Overview

Trinity Healthcare Talent Services is seeking a Travel Occupational Therapist (OT) for a home health role in Sacramento, CA.

This position involves providing therapy services directly in patients' homes, helping improve functional independence, mobility, and ADLs across a post-acute population.

Requirements

• Active California OT License

• BLS Certification (AHA)

• NPI Number required

• Minimum 2 years of healthcare experience required

• Home Health experience strongly preferred

• Valid Driver's License + Auto Insurance (required)

• Must be comfortable working independently in the field

Assignment Details

• Schedule: Monday–Friday | 8:30 AM – 5:00 PM

• Hours: 40 hours/week

• Duration: 13 weeks

• Setting: Home Health

• Coverage Area: Northeast Sacramento to Folsom

• Average Caseload: ~4 visits per day (evals + revisits)

• Floating: First to float as needed

Facility Overview

• Home health division under a major healthcare system

• EMR: Cerner

• Focus on post-acute care and functional recovery

• Combination of evaluations and follow-up visits

• Structured oversight with supervisor check-ins

Important Notes

• Mileage reimbursed (GSA guidelines)

  • Must track and submit signed timesheets

  • Supervisor approval required (bi-weekly or via email)

• 2 supervisor references required at submission

  • 1 within last 12 months

  • 1 within last 3 years

• Profiles are sent directly to the unit (fast process)

• Facility may cancel up to 4 shifts per contract

• COTA may be considered if no OT applicants available
