Schedule: Day Shift | 5x8 Hours (8:00 AM – 5:00 PM) | 40 Hours Guaranteed
Position Overview
Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health role in Washington, DC.
This is a field-based position where the Physical Therapist will provide in-home rehabilitation services across assigned zip codes within the District. The role focuses on improving mobility, restoring independence, and delivering patient-centered care through individualized, evidence-based treatment plans.
The therapist will work independently while coordinating closely with interdisciplinary teams.

Service Area: Washington, DC (assigned zip codes)
Clinical Coverage Areas
Therapists will provide care for:
Adult and geriatric patients
Post-surgical recovery
Chronic conditions (COPD, CHF, etc.)
Mobility and functional impairments
Orthopedic and general rehabilitation needs
Key Responsibilities
Perform in-home patient evaluations and functional assessments
Develop individualized treatment plans based on patient needs
Deliver therapeutic exercises, mobility training, and rehabilitation interventions
Complete OASIS documentation accurately and timely
Manage a caseload of approximately 5–6 patients per day
Monitor patient progress and adjust care plans accordingly
Educate patients and caregivers on home exercise programs and safety
Coordinate care with interdisciplinary healthcare teams
Maintain accurate and compliant clinical documentation
Travel between assigned patient locations efficiently
Required Qualifications
Minimum 2 years of Home Health experience
Active DC or Maryland Physical Therapist License
BLS Certification
OASIS and HomeCare HomeBase (HCHB) experience required
Valid driver's license and reliable personal vehicle
Strong communication and documentation skills
Preferred Qualifications
Previous travel experience
Experience managing independent caseloads
Familiarity with diverse urban patient populations
Scheduling & Assignment Details
40 hours guaranteed per week
Monday – Friday schedule
Shift: 8:00 AM – 5:00 PM
No weekend or on-call requirements
May float between zip codes within service area
Important Notes
Local candidates within 60 miles accepted at a lower rate
Mileage reimbursement: $0.58/mile (from first to last patient visit)
Reliable transportation required for all assignments
Candidates must be comfortable traveling within assigned coverage zones
Interview process may not be guaranteed (clinical review required)
RTO Policy:
- No RTO within first 2 weeks
- Maximum 7 days per contract
- Must be submitted at time of application
Compliance Requirements
Candidates must complete all onboarding and compliance requirements including:
Active state licensure verification
BLS certification
Two supervisor references
Skills checklist and clinical review
Pre-start modules (billable up to 4 hours)
Labs and drug screening within 48 hours of contract signing
