Position Overview
Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a rehab/LTC setting in Humansville, MO.
The Physical Therapist will evaluate and treat patients with physical impairments, mobility limitations, and wound care needs. This role focuses on restoring functional independence, improving mobility, and enhancing patient outcomes through individualized, evidence-based treatment plans.
The therapist will collaborate closely with interdisciplinary teams to ensure high-quality patient care while maintaining compliance with clinical and regulatory standards.

Clinical Coverage Areas
Therapists will provide care for:
Post-surgical rehabilitation
Mobility and functional impairments
Wound care management
Strength, balance, and endurance training
Long-term care and recovery support
Key Responsibilities
Evaluate and treat patients with physical function disorders and/or wound care needs
Develop and implement individualized treatment plans using evidence-based practices
Administer therapeutic interventions including manual therapy, ambulation training, exercises, and modalities
Monitor patient progress and modify care plans accordingly
Maintain accurate and compliant clinical documentation
Educate patients, families, and caregivers on treatment and recovery plans
Collaborate with interdisciplinary healthcare teams
Prepare discharge summaries and coordinate follow-up care
Participate in patient care conferences and rehabilitation meetings
Supervise PTAs, aides, and students when applicable
Required Qualifications
Minimum 1 year of Physical Therapist experience
LTC or Rehab experience required
Active Missouri Physical Therapist License
Strong communication and documentation skills
New graduates are accepted only with Level II clinical experience.
Preferred Qualifications
Experience in skilled nursing or long-term care settings
Wound care experience
Familiarity with electronic documentation systems
Scheduling & Assignment Details
40 hours guaranteed per week
Monday – Friday schedule
Flexible shift timing (must remain consistent and coordinated with DOR)
No weekend requirements
No on-call duties
Important Notes
Consistent schedule communication with Director of Rehab required
Candidates must be comfortable working in rehab/LTC environments
Strong adaptability in patient care planning is essential
Compliance Requirements
Candidates must complete all onboarding and compliance requirements including:
Active state licensure verification
Clinical competency validation
Completion of required training and documentation
