LCSW | LMHC | LMFT | Travel-Based | Post-Acute Care | Regional Lead
Location: Regional / Field-Based (Multi-Site Travel Required) Employment Type: Full-Time Department: Behavioral Health Integration (BHI)
About the Role
The Behavioral Health Case Manager (BHCM) is a licensed clinician who provides clinical oversight, intake assessments, and team leadership across multiple post-acute care facilities within an assigned region. This is a travel-based position -- clinicians drive between skilled nursing and post-acute settings on a regular schedule and must have reliable transportation and a valid driver's license.
BHCMs conduct behavioral health intakes and psychosocial assessments, support Value-Based Care initiatives, and serve as the clinical escalation point for Behavioral Health Coordinators (BHCs). The role blends direct clinical work with team leadership and operational accountability.
