About the Role

The Behavioral Health Case Manager (BHCM) is a licensed clinician who provides clinical oversight, intake assessments, and team leadership across multiple post-acute care facilities within an assigned region. This is a travel-based position -- clinicians drive between skilled nursing and post-acute settings on a regular schedule and must have reliable transportation and a valid driver's license.

BHCMs conduct behavioral health intakes and psychosocial assessments, support Value-Based Care initiatives, and serve as the clinical escalation point for Behavioral Health Coordinators (BHCs). The role blends direct clinical work with team leadership and operational accountability.

This position is a strong fit for clinicians who enjoy systems-level impact, interdisciplinary collaboration, and helping shape a growing behavioral health program.

Key Responsibilities

Clinical Assessment & Intakes

  • Conduct behavioral health intakes and comprehensive psychosocial assessments
  • Perform diagnostic clarification and risk stratification
  • Determine program eligibility and level of intervention
  • Engage patients and families as clinically appropriate

Regional Team Leadership

  • Provide clinical guidance and escalation support to BHCs
  • Review documentation for quality, compliance, and consistency
  • Support onboarding, coaching, and workflow execution
  • Maintain regional coverage and case flow

Care Coordination & Collaboration

  • Collaborate with psychiatry, psychology, and medical providers
  • Participate in interdisciplinary care planning
  • Support transitions of care and discharge planning

  • Timely completion of intakes and assessments
  • Clinical quality and documentation compliance
  • Effective support and escalation resolution for BHCs
  • Regional workflow stability and case progression
  • Strong collaboration with facility and provider partners

Performance expectations are supported through structured onboarding and supervision.

Qualifications

Required

  • Active, unrestricted license: LCSW, LMHC, or LMFT
  • Master's degree in Social Work, Counseling, Psychology, or related field
  • Medicare-eligible for intake and assessment services
  • Minimum 3 years of behavioral health or case management experience
  • Valid driver's license and reliable personal vehicle
  • Willingness and ability to travel regularly between facilities within an assigned region

Preferred

  • Post-acute, SNF, or geriatric experience
  • Value-Based Care or BHI program experience
  • Prior team lead or supervisory experience
  • Bilingual a plus
