Job Description:
Community Education and Healthcare Access:

Collaborates with community members, stakeholders, private and public organizations, and government agencies to improve community health.

Facilitates evidence-based programs (e.g., diabetes prevention, fall prevention, chronic disease self-management) to promote positive lifestyle behaviors and safety.
Community Screenings and Immunizations:

Attends screening and immunization programs, which may occur on evenings and weekends, alongside team members, providers, volunteers, and community partners.

Administers community-based vaccinations (e.g., flu shots, COVID vaccines).

Maintains accurate documentation, vital records, infection control standards, and a safe environment.

Responds effectively in emergency situations, including initiating CPR and calling 911.
Outreach Responsibilities:

Attends community events, including evenings and weekends, to educate on health topics.

Acts as a community health representative in conjunction with department team members.
Community Health Per Diem Pool Guidelines:
-
Schedules are considered a commitment; events canceled without seven (7) days notice count as an occurrence. Two occurrences in six months may result in removal from the pool.

Must work a minimum of two events per month and at least two cholesterol events per year.

One shift per quarter must be a weekend event (four events per year).

CPR certification is required.

Must maintain current Maryland licenses and certifications as applicable.
Qualifications:

Licensure as a Registered Nurse in Maryland, or eligible under Nurse Compact agreements.

12 years of relevant experience in community health (preferred).

Current CPR certification.
-
Valid State Drivers License.
