Position Summary
We are seeking a dedicated Mental Health Therapist to join an integrated, interdisciplinary care team within a primary healthcare setting. This role supports individuals experiencing behavioral health challenges through crisis intervention, ongoing therapeutic services, and the development of individualized behavioral change plans. The Therapist will provide evidence-based treatment to patients with complex medical and psychosocial needs while collaborating closely with primary care providers and other clinical team members to ensure coordinated, patient-centered care.
Key Responsibilities

Diagnose, treat, and manage patients with mental health disorders and psychosocial concerns in collaboration with primary care providers

Provide therapeutic interventions for both acute and chronic emotional, behavioral, and health-related conditions

Conduct comprehensive intake assessments and deliver therapy using evidence-based modalities such as CBT, ACT, DBT, and other approved treatment approaches

Provide individual, couples, family, and group therapy as clinically appropriate

Assess and support at-risk patients by developing prevention strategies and coordinating with external agencies (e.g., DHS, Tribal services, emergency services, hospitals) as needed

Provide emergency telephone counseling and crisis support to patients and care partners

Make referrals to specialty behavioral health or medical providers when clinically indicated

Document all patient interactions, including phone encounters, in a timely, accurate, and compliant manner

Coordinate follow-up care with primary care providers, Psychiatric Nurse Prescribers, and other members of the care team
Education & Licensure

Masters degree in Psychology, Counseling, Social Work, or a related behavioral health field (required)

Active Oregon licensure as LCSW, LMFT, or LPC (strongly preferred)

Licensure supervision available for qualified candidates not yet independently licensed
Preferred Qualifications

CADC I, II, or III certification in addition to therapy licensure

Strong multicultural competency; Spanish-speaking ability highly preferred

Experience in behavioral health medicine and evidence-based treatment models

Ability to complete accurate DSM-5 assessments using standardized diagnostic tools

Proven ability to function effectively in a fast-paced, high-stress interdisciplinary care environment

Proficiency with computers and electronic medical records (EMR) systems
