Pay Range: $35.00 – $50.00 per hour (based on relevant experience)

Position Overview
WellTech partners is seeking an experienced and compassionate Director of Social Services to lead and oversee the psychosocial care of residents within our clients facility. This role plays a critical part in supporting residents and families through assessment, care planning, crisis intervention, and discharge planning, while ensuring compliance with all state and federal regulations.
The ideal candidate is a strong communicator, highly organized, and knowledgeable in MDS, care planning, and interdisciplinary collaboration.
Key Responsibilities

Assess, identify, and document residents medically related social, emotional, and psychosocial needs

Coordinate care with residents, families, interdisciplinary team members, and community resources

Provide crisis intervention and emotional support to residents and families as needed

Lead and manage discharge planning, ensuring smooth transitions of care

Ensure accurate and timely documentation for MDS and Care Plans

Provide in-service education to staff on residents social and emotional needs

Supervise and support the Social Services Assistant

Maintain compliance with state, federal, and corporate guidelines
-
Build and maintain strong relationships with referral agencies, service providers, and community partners

Attend required staff, department head, corporate, and social services meetings

Perform additional duties as assigned
Qualifications

Master of Social Work (MSW) – Required

Bachelors degree in Social Work or Gerontology – Preferred

Working knowledge of MDS, care planning, and discharge planning

Strong verbal and written communication skills

Ability to work collaboratively with residents, families, and interdisciplinary teams

Knowledge of healthcare regulations and resident rights
