Registered Nurse (RN) – DP/SNF | Full Time
Pay Range: $46.47 – $65.05 per hour
Shift: Variable 12-hour shifts
Bonuses:

Position Summary
We are seeking a dedicated and skilled Registered Nurse (RN) to join our client's DP/SNF (Distinct Part Skilled Nursing Facility) team. In this role, you will provide comprehensive, patient-centered nursing care to adult and geriatric populations (21+), utilizing the nursing process to assess, plan, implement, and evaluate care.
The RN demonstrates strong clinical judgment, leadership, and accountability while ensuring compliance with regulatory standards, patient safety guidelines, and nursing best practices.
Patient Population
- Adult (21+) and geriatric patients
- Residents requiring skilled nursing and long-term care services
- Interaction with families, physicians, staff, and the broader community
Key Responsibilities & Essential Functions
Patient Assessment & Care Planning
- Perform comprehensive and ongoing patient assessments, including physical, psychosocial, environmental, and educational needs
- Develop, implement, and regularly update individualized care plans based on patient status and goals
- Identify nursing diagnoses and interdependent care needs
- Recognize subtle or early signs of clinical deterioration or complications
Clinical Care & Treatment
- Deliver safe and effective nursing care in accordance with clinical policies and standards
- Administer medications safely and demonstrate understanding of drug interactions, especially for multi-system conditions
- Initiate, monitor, and discontinue IV therapies, including TPN and (where applicable) chemotherapy
- Provide care for central lines (e.g., PICC lines, porta-caths)
- Perform respiratory and general clinical assessments
- Troubleshoot equipment and respond to patient care needs promptly
Care Coordination & Communication
- Communicate changes in patient condition to physicians and interdisciplinary teams
- Act as a liaison between patients, families, and healthcare providers
- Participate in shift reports and care transitions, ensuring continuity of care
- Delegate tasks appropriately to LVNs, aides, and support staff
Evaluation & Documentation
- Evaluate effectiveness of care plans and revise based on patient outcomes and progress
- Maintain accurate, timely, and compliant clinical documentation
- Ensure documentation aligns with hospital policies, regulatory standards, and patient safety goals
Patient & Family Education
- Provide and evaluate patient and family education, adjusting teaching methods as needed
- Support discharge planning and long-term care education
- Promote patient engagement in care and safety practices
Quality, Safety & Compliance
- Adhere to National Patient Safety Goals (NPSGs) and infection prevention protocols
- Follow OSHA guidelines and universal precautions
- Identify and report quality or risk management concerns
- Participate in quality assurance and performance improvement initiatives
Leadership & Team Support
- Serve as a clinical resource for peers, LVNs, aides, and students
- Assist with orientation and training of new staff
- Support or assume Charge Nurse responsibilities as needed
- Participate in unit meetings, problem-solving, and process improvements
Professional Practice
- Maintain current knowledge of healthcare trends and long-term care regulations
- Uphold the California Nursing Practice Act and ANA Code of Ethics
- Demonstrate accountability, professionalism, and commitment to excellence in nursing practice
Qualifications
Education & Experience
- Minimum 1 year RN experience in:
- Acute Care or
- Skilled Nursing Facility (SNF) (Preferred)
Licensure & Certifications
- Active California Registered Nurse (RN) License (Required)
- BLS Certification (Required)
Skills & Competencies
- Strong clinical assessment and critical thinking skills
- Ability to manage complex, multi-system patient needs
- Effective communication and team collaboration
- Knowledge of long-term care regulations and resident rights
- Strong organizational skills and ability to prioritize in a fast-paced environment

Work Environment & Expectations
- Must be able to work variable 12-hour shifts, including weekends, holidays, and on-call
- Expected to participate in emergency and disaster response situations
- Demonstrates flexibility with staffing needs and changing patient volumes
