Pay Range: $35.00 $47.50 per hour depending on experience

About the Role:
We are seeking an experienced MDS Coordinator to join a well-respected skilled nursing facility in Boulder City, Nevada. This position plays a critical role in ensuring accurate resident assessments, optimizing reimbursement, and driving quality of care through proper documentation and planning. The ideal candidate is detail-oriented, proactive, and confident in leading staff toward best practices in documentation and care planning.
Key Duties & Responsibilities:

Coordinate and manage all aspects of the Minimum Data Set (MDS) process, ensuring compliance with state and federal guidelines.

Develop and maintain comprehensive, person-centered care plans for residents in collaboration with the interdisciplinary team.

Review documentation to identify gaps or inconsistencies and coach nursing staff toward improvement.

Monitor the accuracy and timeliness of assessments and submissions to support optimal reimbursement.

Conduct internal audits and participate in quality improvement initiatives.

Act as a resource and educator on MDS processes and care plan development for nursing and clinical staff.

Assist with survey readiness and respond to documentation-related inquiries during regulatory reviews.

Collaborate with department heads and leadership to evaluate and enhance care delivery systems.
Qualifications:
-
Previous experience as an MDS Coordinator in a skilled nursing facility is required.

Strong understanding of the RAI process, PPS, Medicare/Medicaid regulations, and case mix methodologies.

Proficiency in care plan development, documentation audits, and CMS guidelines.

Excellent interpersonal, leadership, and communication skills able to coach and collaborate effectively.

RN or LVN/LPN licensure in the state of Nevada (or ability to obtain) preferred.

Detail-oriented with strong organizational and time-management abilities.

EMR/MDS software proficiency is a plus.
