We are seeking a passionate and dedicated Physical Therapist (PT) to join our clients rehabilitation team in Alameda, CA. The ideal candidate will provide exceptional patient care focused on restoring and enhancing independent function for individuals impacted by illness or injury. Key Responsibilities

  • Conduct thorough patient evaluations and develop individualized treatment plans.

  • Deliver high-quality therapeutic interventions to help patients achieve functional goals.

  • Supervise Physical Therapy Assistants and Aides in accordance with facility policy and state regulations.

  • Maintain detailed, accurate documentation including evaluations, treatment notes, progress reports, and discharge summaries.

  • Collaborate with interdisciplinary teams, families, and physicians to ensure coordinated, patient-centered care.

  • Educate patients, families, and facility staff on therapy goals, exercises, and care plans.

  • Uphold safety standards and infection control procedures in all work areas.

  • Participate in professional development, continuing education, and in-service training to maintain clinical excellence.

Qualifications

  • Bachelors, Masters, or Doctorate degree in Physical Therapy from an accredited program.

  • Current Physical Therapist License in the State of California.

  • Current CPR certification required.

  • Strong clinical assessment, documentation, and communication skills.

  • Experience in geriatric rehabilitation or adult neurological/cardiac/general rehab preferred.

  • Ability to supervise assistants and aides effectively.

  • Commitment to compassionate, patient-centered care.

Physical Requirements

  • Ability to lift and transfer patients (50 -70 lbs) multiple times per day.

  • Frequent standing, walking, bending, and kneeling (6080% of workday).

  • Manual dexterity and physical stamina required for direct patient care.
