About the Role
We're seeking a compassionate and dedicated Occupational Therapist to join our clients skilled nursing and rehabilitation team in Alameda, CA. In this role, you'll help patients restore and enhance their independence, working closely with an interdisciplinary team to deliver personalized, goal-oriented care.

Key Responsibilities

Conduct comprehensive evaluations and develop individualized treatment plans tailored to each patients functional goals.

Deliver therapeutic interventions to improve patients independence in daily living activities (ADLs).

Supervise and mentor Certified Occupational Therapy Assistants (COTAs) and Aides, ensuring compliance with facility, state, and federal standards.

Document all evaluations, treatments, and progress notes accurately and in accordance with regulatory requirements.

Collaborate effectively with patients, families, and interdisciplinary team members to ensure continuity of care.

Educate patients and caregivers on exercises, adaptive equipment, and safety practices to support successful home and facility transitions.

Maintain a safe, organized, and positive therapy environment.

Participate in case conferences, quality improvement initiatives, and continuing education to maintain clinical excellence.
Qualifications
-
Bachelors or Masters degree in Occupational Therapy from an accredited program.

Current California Occupational Therapist License required.

Current CPR certification.

Excellent verbal, written, and interpersonal communication skills.

Experience in geriatric rehabilitation or adult neurological/cardiac/general rehab preferred.

Strong documentation, assessment, and time management abilities.
Physical Requirements

Ability to lift and transfer patients (50-70 lbs) multiple times per day.

Frequent standing, walking, bending, and kneeling (6080% of the workday).

Manual dexterity required for patient care and documentation.
