We are hiring an Intake Coordinator / Patient Care Specialist to manage client onboarding, caregiver coordination, and day-to-day scheduling for in-home care services. This role focuses on organizing intakes, matching clients with caregivers, and keeping operations running smoothly.

No prior home care experience is required. Candidates with strong administrative, HR, or customer service backgrounds are encouraged to apply.

Key Responsibilities

  • Manage the full intake process for new clients from initial contact to service start
  • Match clients with appropriate caregivers based on needs and availability
  • Coordinate schedules, shift coverage, and new case assignments
  • Maintain communication with clients and caregivers to resolve issues quickly
  • Support caregiver onboarding, documentation, and compliance requirements
  • Assist with recruiting, screening, and coordinating interviews for caregivers
  • Keep accurate records in company systems and ensure data is up to date
  • Monitor active cases and ensure consistent service delivery

Qualifications

  • Experience in administrative support, customer service, or HR
  • Strong organization and ability to manage multiple tasks at once
  • Clear communication and problem-solving skills
  • Comfortable using Microsoft Office and general software systems
  • Experience with tools like HHA Exchange, Smartsheet, or Adobe is a plus
  • Able to work independently and stay on top of tasks without supervision
