Responsibilities

·       Provide coordination and care management for assigned patients

·       Connect patient with community resource and financial assistance

·       Perform nursing procedures in home environment

·       Develop and evaluate care plan and goals

·       Liaise with other healthcare partners who are caring for the same patient

·       Participate in multidisciplinary round

·       Maintain timely and accurate documentation of patients’ condition and care given

·       Provide care giver training programme and health education to patients / caregivers and evaluate the teaching programmes to meet patients and caregivers needs and expectations.

·       Participate in quality improvement project and nursing research

Job Requirements

• Registered Nurse with Singapore Nursing Board
• Minimum 3 years’ experience working in acute hospital settings and or familiar with Community hospital settings
• Candidates with Advance Diploma in nursing specialties preferred
• Able to be work on extended hours (5pm to 9pm) on certain weekdays, on rotational basis,estimates 3 to 4 times a month
• Must be comfortable to travel around on a daily basis
• Head office: Toa payoh
• Monday to Friday
