 Job Description:

Implement Care Coordination programme activities and workplans, including piloting joint referral workflows and care coordination initiatives to support integrated care delivery.

Conduct interRAI care assessments and undertake care planning and coordination responsibilities, as assigned.

Serve as the primary point of contact for partner organisations within the assigned sub-region(s), building and maintaining strong working relationships to facilitate service integration.

Maintain a strong understanding of the services and offerings, and proactively identify opportunities to bridge and align services across participating sub-regions.

Represent the organization in ICCP sub-regions, ensuring alignment with organisational objectives and fostering effective partnerships.

Support community outreach efforts to build and strengthen partnerships, raise awareness of ICCP initiatives, and facilitate collaboration with community stakeholders in the sub-region.

Organise and support ICCP Steering Committee and operational meetings, including agenda preparation, minute-taking, performance reviews, and follow-up actions to drive strategic and operational alignment.

Act as a key liaison between internal teams and external partners to ensure clear communication, effective collaboration, and smooth execution of joint initiatives.

Monitor service delivery performance, track progress against targets, and support the resolution of operational challenges.

Identify service gaps and opportunities for improvement, and support the implementation of best practices to enhance service quality across partner organisations.

Assist in the preparation of reports, dashboards, and presentations for internal management review and external reporting, as required.

Support the management in maintaining oversight of ICCP sub-regional involvement, including tracking developments, milestones, and status updates.

Qualifications

  • Degree in any social work, nursing or healthcare-related discipline would be an advantage
  • Possesses strong interpersonal communication and stakeholder management skills to coordinate these care needs for the elderly seniors in our sub-regions with our network of providers available in the sub-region. The incumbent will be reporting to the Head of Regional Services and is expected to able to work effectively in a fast-paced environment.
