Job Role

• To assist new patients with their applications and Know Right Start Right project
• Degree or equivalent in any disciplines

Main Responsibilities

• First contact for HD applicants
• Process new applications for the HD programme
• Conduct household Means Test and determine the subsidy quantum and welfare assistances
• Admissions Intake assessment of applicants Risks and Needs profile and referrals for assistance
• Create and update patient fee structure, welfare applications & Admissions intake in the system
• New patient DC Orientation
• Home visits
• Respond to MP letters/emails
• General admin duties
• Contact point for all restructured hospitals for HD applications
• Admissions and Social Work case handovers
• Managing of new patients who are deemed to be more challenging
• Attending to Know Right Start Right referrals
• Contribute and ensure that the department meet its KPIs

Qualification (Related Field)

• Degree or equivalent in any disciplines

Experience

• 1-3 years experience in social service or healthcare industry

Soft Skills

• Dynamic, self-motivated and patient-oriented
• IT Savvy
• Team player
