Manage the Portable Funding (PF) and Enhanced Potable Funding (EPF) Programmes.

Main Responsibilities

• Process new applications for PF/EPF.
• Conduct financial Means Test and determine the subsidy quantum for standard cases.
• Conduct yearly financial assessment on eligibility for Government and the client's subsidies programmes.
• Case manage the changes for PF/EPF programmes.
• Ensure that existing cases are reviewed before the expiry of subsidies.
• Liaise with social workers and renal coordinators of restructured hospitals of the patients application status, financial and psychosocial issues.
• Identify and place patients into private dialysis centre based on available capacity.
• Slot management and analyse utilisation and occupancy trends.
• Track and optimise the utilisation of capacity of dialysis centres and recommend resource or load balancing measures in right-siting slots.
• To generate reports from different systems to assist with internal and external audit checks.
• Document bio-data of patients and requests in case-files for record.
• Provide regular and ad-hoc reports on the both programmes.
• Prepare letters and ensure that the patient information and data are updated in a timely and accurate manner.

Experience

• 2 year experience in social service or healthcare industry
