Need 2 headcounts for 5 months contract, convertible to permanent headcount.

Responsibilities and Duties:

• Perform social and financial assessment of patients and make recommendations.
• Provide counselling to patients and their families and lead in family conferences.
• Liaise with relevant organisations for collaboration and networking in care of the elderly.
• Attend to the social needs of destitute patients.
• Participate in a multi-disciplinary team on patient-care.
• Conduct groupwork with the elderly patients or their caregivers.
• Implement initiatives and support programmes that enhances the care and support of elderly patients and their caregivers within the community

Working hours:

• Monday to Thursday : 8:30am to 5:45pm
• Friday : 8:30am to 5:30pm
• One Saturday per 4 weeks: 8.30am to 12.30pm
• Total: 42 hours per week
