Responsibilities:

• Provide high quality patient-centered care
• Conduct training and support for caregivers at home
• Administer medication and treatment as prescribed and agreed upon by physician
• Provide emotional support and counselling to patients and their families where necessary
• Educate patients and their caregivers on symptom management and disease progression
• Plan, schedule and manage resources for effective home visits
• Proficient in conducting initial, functional and cognitive assessments and others in accordance to MOH service guidelines
• Coordinate, implement and review individualized care plans every 6 monthly
• Document visits and encounters in accordance with organisational policies
• Coaching and sharing of knowledge with fellow team mates
• Provide administrative support to Home Care Service
• Participate in quality improvement initiatives and research projects assigned
• Coordinate with internal and external care partners with involvement in patient care

Requirements:

• Candidate must possess at least a Diploma/Degree in Nursing and valid registration with Singapore Nursing Board
• Minimum 5 years of experience in General Medical, Surgical, and/or Geriatrics discipline
• Clinical experience in Palliative or Community Care nursing with Specialist Diploma will be an added advantage.
• Competent and skillful in clinical tasks like insertion of NGT etc
• A team player with initiative and an excellent service attitude. Able to collaborate effectively with team members and working towards common goals to provide high quality care to patients.
• With excellent interpersonal and communication skills
• Ability to work independently and accountable as part of a team
• Willingness to work flexible hours with work travelling within Singapore
