Responsibilities:

We are seeking a reliable and detail-oriented team member to assist with the following responsibilities:

• Collect and document patient information related to rehospitalizations

• Manage incoming phone calls from hospital departments and respond via team chats

• Support Intake Departments by answering and transferring calls as needed

• Follow up with Primary Care Providers (PCPs) regarding hospice referrals

• Fulfill requests from clinical staff for additional orders and patient medical records

Qualifications:

• Must be a Registered Nurse with a valid PRC license

• Prior BPO experience is an advantage

• Ability to work independently as well as collaboratively in a team environment

• Strong customer service skills and professional communication

• Excellent multitasking and organizational abilities

• Must be available to work overnight (graveyard shift)

• Willingness to work weekends, if needed
