Position Details:
Location: Albuquerque, NM
Employment Type: Permanent, Full-Time
Salary: $85,000 - $110,000 per year (based on experience)
Relocation & Sign-On Bonus: Available for qualified candidates
Territory: Albuquerque and surrounding areas
On-Call: None

• Base Salary: $85,000 - $110,000 per year (based on experience)
• Sign-On Bonus: Available (case-by-case)

Job Summary and Qualifications
The Physical Therapist will provide in-home therapy services to patients, helping them regain mobility, reduce pain, and improve quality of life. The ideal candidate will be self-motivated, compassionate, and a strong communicator.

Responsibilities:

• Provide comprehensive physical therapy evaluations and treatments in the patients home.
• Develop and implement personalized care plans tailored to patient needs.
• Educate patients and caregivers on therapeutic exercises and home safety modifications.
• Monitor and document patient progress in compliance with agency and state regulations.
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes.

What Were Looking For:

• Licensed Physical Therapist (PT) in NM (or eligibility).
• 1+ years of home health or outpatient experience preferred.
• Strong documentation and communication skills.
• Ability to work independently and as part of a team.
• Reliable transportation and valid drivers license.
